How To Become a Patient
How do you register at Los Barrios Unidos Community Clinic (LBUCC) once you have made an appointment?
On the day of your appointment, please bring the following:
We can help you determine if you qualify for insurance programs such as Medicaid, CHIP, CHIP Perinatal or the Marketplace insurance plans during the active enrollment period. If you do not have insurance, or if you do not qualify for insurance, we will determine what percent of the actual clinic cost you will have to pay when you register. We charge a sliding fee scale based on your income and family size. You can also make payment arrangements with the Business Office for sliding fee scale (discounted) payments. The clinic provides care regardless of a person’s ability to pay.
We understand that information about you and your health is personal, and protecting your health information is important to us. The notice found at the link below entitled Notice of Privacy Practices applies to all of the records of your care generated by LBUCC, whether made by LBUCC personnel or other health care providers.
